Program Information
The Home and Community Care Program, along with the Assisted Living Program provides home nursing, personal care and assisted living services for community residents with acute and chronic health conditions.
The goal is to support people to be close to their loved ones as long as possible and to keep their independence.
Currently, the Home and Community Care program employs two full time Registered Nurses, one full time Assisted Living/Administrative Support person, four full time Certified Health Care Aides and three fulltime homemakers.
Services consist of:
Funding for the Home and Community Care Program remains at a level where services beyond Monday to Friday days is not supported, however we make every effort to extend our nursing services beyond that when community members need support for a brief period of time. Example being dressing changes for clients not able to attend hospital on weekends and palliative care management.
Assisted Living In-Home Care (Home Maker Program)
The INAC-funded Assisted Living In-Home Care program (commonly known as the Home-Maker Program) is designed to provide non-medical home support to community members. In addition to home making services, this program also provides summer grass cutting and winter snow clearing for program clients who are unable to manage this themselves and who do not have family able to help.
Client Profile Summary:
The majority of our clients are elderly. Client services this year included nursing care for acute illnesses or convalescence following hospitalization, support with home making and personal care, chronic and acute wound care, and medication management. A small but significant client group with chronic and severe mental health conditions receives nursing services such as regular injections.
Adult Day Program
We are looking to begin an Adult Day Program in late March early April 2022 when the weather warms, COVID-19 cases stabilize and clients are willing to come out and enjoy the activities provided.
The intent is to provide older adults an opportunity to get out of the house and receive mental and social stimulation. The benefits of this program would include a safe environment to spend the day/morning/afternoon, educational activities, a maintained level of independence, socialization and nutritious meals and snacks. HCA and Nurses will inform clients of upcoming events by word of mouth, and flyers/invitations. The program will be provided 2x monthly. Activities may include nutrition presentation, Diabetes presentation, crafts, and painting

- Client assessment
- Case management
- Referrals
- Nursing services (e.g. medication administration, dressing changes)
- Personal care services (e.g. assistance with bathing, grooming, dressing)
- Home management (e.g. light housekeeping, laundry, meal preparation).
- In-home respite care (caring for someone while family members have a rest)
- Record keeping and data collection. Completed by all HCC staff.
- In home palliative care management.
